Case Management services are available to provide information and referral services for residents who are low-income individuals or families needing social services assistance. This service is free to the Chino Community.

Case Management Services are available at the following locations and times:
- Neighborhood Activity Center (5201 D Street)- Tuesday-Friday from 10:00 a.m. to 4:00 p.m. Evening appointments are available upon request.
- Senior Center (13170 Central Avenue)-Make an appointment by calling (909) 334-3271 or emailing communityservices@cityofchino.org.
Community Outreach
Mobile Stop is an outreach program which is provided to the community at various locations in Chino. Types of information available at Mobile Stops inlcude:
- Social Service Information
- Referral Services for the homeless, physically or mentally challenged, elderly, and Spanish speaking residents
