Rudy was experiencing homelessness in Chino, living behind a dumpster when he was first connected with the City of Chino’s Homeless Outreach Program. From the beginning, Rudy expressed a strong desire to rebuild his life and was committed to taking the necessary steps toward stability.

The Homeless Outreach Team provided Rudy with a clear, personalized plan of action. They assisted him in obtaining essential identification documents, including his birth certificate, driver’s license, and Social Security card. With these documents in hand, Rudy was able to access critical benefits and services to begin rebuilding his life.

The team was able to connect Rudy to a 24/7 senior group care facility, where he secured permanent housing through a shared living arrangement. His new living situation also included access to supportive services such as Meals on Wheels, an In-Home Supportive Services (IHSS) worker, and transportation to ensure he could attend his medical appointments.

With the support of the program, Rudy has regained a sense of normalcy and stability. He is now on a path toward long-term self-sufficiency and is actively participating in services that support his housing, health, and overall well-being.

As a senior unable to work, Rudy is focused on maintaining stable housing and accessing the long-term benefits and healthcare services he needs to age with dignity and security.
